Elitist If your bontebok was legally taken, and you have papers on it (from the farm where you hunted and the South African authorities) to show this; if your shipment has been properly prepared, and documented as such, your taxidermist will make arrangements for the shipment. South Africa has endemic Foot & Mouth Disease (FMD) and NOTHING from an FMD country can be sent here without proper treatment, lest this disease be transmitted to US livestock. The F&W Service is concerned about things like endangered species (e.g., elephant) and the Agriculture Department with disease. The form you ask about is essentially a certification that your trophy is not from an endangered/threatened species, and that it has been taken in accordance with SA law. Your outfitter and taxidermist should make these arrabgements. It isn't cheap but it will save you volumes of hassle and get your trophy here in proper condition for importation. If you need a copy of the form contact the Fish & Wildlife Service office near you or in DC; if you need an import permit to certify the trophy has been treated, contact the Animal & Plant Health Inspector (USDA) nearest you. They will be in the phone book under "Government--Federal."
